  • SUMMARY:

    The Health Educator/Care Navigator addresses and coordinates services identified in the care plan developed by the Program Director and/or treating physician. This can include engaging and retaining consumers in care, providing linkages, referrals, and coordination of treatment.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

    1. Monitors and conducts follow-up activities to ensure the service plan is effectively implemented and adequately addresses the client needs.

    2. Follows up with clients and families to support adherence with treatment guidelines.

    3. Maintain regular ongoing contact with the client, their health care providers and other providers, family and other community support to ensure progress on implementing the care plan and resolve any coordination issues encountered.

    4. Consults and works cooperatively with Physicians, Nurses, Administration and other team members to achieve a common goal of continued recovery.

    5. Maintains and utilizes data for evaluation.

    6. Train physicians, patients, medical student, and community on administration of naloxone.

    7. Works with director to develop best practices.

    8. Be an ambassador for the program.

    9. Keeps abreast of all federal, state and Rowan University regulations, laws and policies as they presently exist and as they change or are modified.

    10. Performs other duties as assigned.

    QUALIFICATIONS:

    EDUCATION and/or EXPERIENCE:

    Bachelor's Degree in Health, Psychology, counseling, social work, education, or a related field plus two (2) years of experience in addiction. May be required to follow a varied schedule which may include evenings, weekends and holidays. May be required to possess a valid driver’s license.
